Appointments

Please note: Appointments cannot be booked directly by the patient.

Our practice is dedicated to providing specialized care to patients referred to us by their primary care physician or other medical specialists. This process ensures that your medical history is properly documented, and that our specialized services are the right next step in your treatment plan.

The Referral Process:

  1. Ask your current doctor for a referral.
    • Contact your Primary Care Physician (PCP) or current referring specialist.
    • Explain that you need a referral to our practice for your specific condition.
  2. Your doctor will submit the referral paperwork.
    • Your doctor’s office will send the official referral form, along with any necessary supporting documentation (like recent test results, imaging reports, or consultation notes), directly to our office.
  3. We will contact you to schedule.
    • Once we receive and review your complete referral package, our scheduling coordinator will contact you directly via phone or email to set up your initial consultation.

If you arrive for an appointment without a valid referral on file, we will be unable to see you.

What to Expect After a Referral is Sent

  • Review Time: Please allow 3-5 business days for our team to process the referral paperwork and review the clinical information.
  • Next Steps: We will call you to discuss available dates and times. If there are any missing documents or if we require additional information before scheduling, we will contact your referring doctor’s office.

We look forward to partnering with you and your current medical team to provide you with expert, specialized endocrinology and diabetes care.

Welcome back! We’re happy to help you book your next visit with us.
